Thank you for your interest in PHI. Included below are instructions for completing our on-line application. Please read them thoroughly. You will go to a new page to complete the application so you may want to print them out as well. When you are ready to complete the application click "go to APPLICATION FORM" above.

• You need to have Adobe Acrobat Reader installed on your computer to open the PHI job application. If you do not have Adobe Acrobat Reader on your machine click Download Adobe Acrobat Reader on the left menu to download the software for free.
• Tab through the fields, the cursor will look like an "I" when you are able to type and the mouse will look like a pointing finger when you are able to click.
• Click the mouse or press the space bar to select the appropriate square boxes (found in Yes / No questions and those where multiple selections are possible).
• You can click the "Zoom In Tool" button (it looks something like this ) to make the application form easier to read.
• When identifying the facility to which you are interested in applying, select that facility from a list of drop-down choices by clicking on the down-facing arrow (Follow the same steps to identify the facility where you previously worked also, if applicable).
• REQUIRED fields are indicated by a RED border.
• Dates should be formatted as 1/1/2002.
